Your online medical record

Login into NHS online services or get the NHS App to access your medical record online

You do not need to come into the surgery to register, just follow the links above and you will be taken through the registration and ID verification process.

The NHS App and website allow you to access a range of services via your computer, mobile or tablet. Once you have signed up, you will be able to

  • renew or order repeat prescriptions online

  • cancel an appointment or book some routine appointment, such as cervical smears

  • view your health record, including your medication, allergies, vaccinations, previous illnesses, letters, consultation notes and test results.

Need to see more information

From the switch-on in Autumn 2023 patients have been able to see all their test results, consultation notes and letters via the NHS App. But they have not automatically had access to their records from before this date, and if they have changed surgeries, their online access does not automatically follow them.

If you need to see older records, or more records than you can at the moment, then please contact the surgery and explain exactly what you need.

 

Accessing the online medical record for a child, or an adult who lacks capacity

If you have legal parental responsibility for a child you can request proxy access to their record. Depending on the child's age and capacity, the child may have to give consent for this.

You can also request proxy access to the medical record of another adult if the adult has given their consent and they lack capacity to access and use their online record themselves. 

To request proxy access you must have an NHS online account yourself so follow the links above if you do not already have one. Then contact the surgery.

For understandable reasons the practice needs to be satisfied that the proxy access will not be used against the interests of the child or adult.

For this reason, a request for proxy access is not an automatic process and the practice will consider your request carefully and individually.
