New Patient Registration 

registrationIf you live in our practice area and would like to register with us our reception staff will be happy to guide you through the procedure.  Please complete and sign one of our registration forms (GMS1) and a New Patient Questionnaire which will provide useful information whilst we wait for your medical records to arrive from your previous doctor.  Please also provide photo ID and proof of address.

These forms can be completed and dropped into reception, posted or emailed to the practice -

Please bring in an up to date prescription request form and make sure you have asked for 1 month's medications from your previous surgery. 

Note that by sending the form you will be transmitting information about your self across the Internet and although every effort is made to keep this information secure, no guarantee can be offered in this respect.

All new patients may be asked book a health check with a member of the healthcare team if they wish in order to ensure that any required tests are up to date and that we have an accurate note of any repeat medication you many be taking.

Medical treatment is available from the date of registration.

On the form, you will be able to say which practitioner you would prefer to see. However, you will be registering with the practice rather than an individual GP.
